différents processus cognitifs, et notamment l'élaboration des représentations de
la verticale (cadre de référence allocentré) et des axes du corps (cadre de référence
égocentré). Nos études sur la verticale subjective visuelle montrent que les patients
négligents présentent, en roulis, une déviation qui s'aggrave dans l'espace négligé.
Cette déviation anti-horaire s'avère fortement modulée par des troubles associés tels
que le syndrome pusher ou l'hémianopsie. En tangage, une inclinaison postérieure de
la verticale subjective est décrite pour la première fois. Concernant la représentation
erronée de l'orientation du corps, nos travaux sur le droit devant subjectif visuel
confirment que la négligence spatiale se caractérise par une déviation ipsilésionnelle
de l'espace égocentré et indiquent qu'il s'agit d'une translation plutôt que d'une
rotation. Cette erreur en translation s'avère plus forte pour le tronc que pour la
tête. Ces éléments nous conduisent à interpréter la déviation latérale classique du
droit devant subjectif comme l'expression d'un trouble multidimensionnel de la représentation
de l'axe principal du corps, se traduisant par une inclinaison anti-horaire. Ils suggèrent
aussi un lien avec la représentation erronée de la verticale. Dans une dernière série
d'expériences consacrée à l'influence des systèmes sensoriels sur ces désorientations,
des effets positifs sont obtenus avec des manipulations posturales et des stimulations
vestibulaires galvaniques. En conclusion, les troubles spatiaux des patients négligents
sont multidimensionnels. Leur expression est fortement influencée par les troubles
visuels ou posturaux associés ; elle peut être modulée en modifiant l'activité de
systèmes afférents
Note publique d'information : Unilateral neglect affects various sensory systems and various cognitive processes,
in particular the internal representation of the vertical (allocentric frame of reference)
and that of the body (egocentric frame). Our studies on the visual subjective vertical
showed that neglect patients deviated in roll plane, more in the neglected space.
This anti-clockwise deviation was modulated by associated disorders (pusher syndrome
or hemianopia). In pitch plane, a posterior deviation of the subjective vertical was
observed for the first time. Concerning the reprentation of body orientation, we confirmed
that unilateral neglect is characterized by an ipsilesional deviation of the visual
subjective straight-ahead and showed that this error resulted from a translation of
the egocentric frame of reference rather than from a rotation. This translation proved
to be greater when the anchor for the task was the trunk than when it was the head.
These data led us to hypothesize that the classic lateral devation of the subjective
straight-ahead is the expression of a multidimensional disorder of the representation
of the main body axis resulting in an anti-clockwise deviation. They also suggested
a link with the erroneous representation of the vertical. A last series of experiments,
devoted to the influence of the sensory systems on these disorientations, showed positive
effects of postural manipulations or galvanic vestibular stimulations. In conclusion,
the spatial disorders of neglect patients are multidimensional. They are strongly
influenced by associated visual and postural troubles and can be modulated by modifying
the activity of afferent systems
